What is Cadence? Here's how we're transforming remote care
In a new video, company leaders, patients, and health system partners discuss Cadence’s approach to Remote Patient Monitoring (RPM) and explain the infrastructure it is building for healthcare systems to deliver remote care to patients all across the United States.
Cadence recently announced a Series B funding round to help physicians deliver more personalized, responsive and data-driven care to millions of patients across the country.
Since its founding, the company has developed a state-of-the-art platform to deliver transformative, remote care to patients with chronic conditions, such as heart failure, type 2 diabetes, hypertension, and chronic obstructive pulmonary disease.
“I founded Cadence because there's a huge opportunity to help millions of people get better treatment for their chronic conditions and to help thousands of clinicians deliver world class care to patients all across the U.S.," said Cadence CEO and Founder Chris Altchek in the video.
Once enrolled, Cadence patients receive a set of smart devices and upload their weight and vital signs on a daily basis, all from the comfort of their own home. That data gets collected, synthesized, and analyzed by a team of Cadence clinicians who work to monitor the data and ensure that patients are staying healthy.
"What's really exciting is bringing guideline, evidence-based medicine to the rural areas of the country," said Cadence Advanced Heart Failure Nurse Practitioner Ana Patsiornik. "Patients [are] receiving the very best care right in their homes without having to drive many of them miles and miles to get to the nearest destination care."
Beyond patients, Cadence also makes it easier for clinicians to deliver better health outcomes. Cadence has partnered with some of the largest hospital systems in the country, including LifePoint Health. The company has developed a new model of integrated care combining in-person care, virtual care, and ongoing patient monitoring.
"We look at Cadence as a tool and a service that we can bring to the benefit of our providers and our patients, and that we're giving another tool in the toolbox to assist those providers in doing the highest quality, safest care for patients with chronic illnesses,” said Dr. Christopher Rehm, Senior Vice President and Chief Medical Officer of LifePoint Health.
