Cadence’s 2024 Outcomes Report: A New Era in Primary Care
This year at Cadence we have worked to redefine primary care by moving beyond monitoring to a more active form of care, ensuring that patients receive comprehensive support when and where they need it.
Our newly released 2024 Outcomes Report, A New Era in Primary Care, highlights the strides we’ve made in addressing the challenges of a strained healthcare system, equipping patients, providers and systems with the tools, team, and infrastructure needed to deliver life-changing, whole-person care.
While we have a ways to go in making the impact we know possible, we are proud to share our progress this year with our community. Here are some of the report's key takeaways:
1. From Monitoring to Care
With primary care under immense strain and a projected shortage of up to 86,000 physicians by 2036, the need for innovative solutions is more urgent than ever.
Cadence’s Remote Patient Care model includes daily monitoring, medication optimization, and personalized care plans tailored to each patient’s unique needs. As Dr. Marat Fudim, Advanced Heart Failure Specialist and Cardiologist at Duke Health explains: “Remote Patient Care isn’t about replacing in-person care. Instead, it complements traditional care by helping physicians focus on the most relevant clinical data over time, ensuring better patient outcomes.”
Our approach has shown we can not only achieve better clinical outcomes for those with chronic conditions, but also potentially enhance care for a broad spectrum of older Americans, supporting their health more comprehensively than ever before, and setting a new standard for primary care.
2. Eradicating Care Deserts
Nearly 60% of Cadence patients live in rural or underserved areas where access to primary care is often limited. Through partnerships with leading health systems like Lifepoint Health, Remote Patient Care has offered a lifeline for these underserved populations, providing timely and accessible care to those who would otherwise have to wait for treatments. We have also seen these patient populations achieve health outcomes equal to or better than their urban counterparts:
- Patients enrolled in the Cadence diabetes program living in underserved areas achieve a target blood glucose (<154mg/dL) at a rate that is 10% higher than their urban counterparts (underserved: 63% vs. urban: 53%).
- Patients enrolled in the Cadence hypertension program living in underserved areas achieve a goal blood pressure (<130/80mmHg) at a 1% higher rate than their urban counterparts (underserved: 35% vs. urban: 34%).
- Over 27,000 remote encounters happening outside of business hours in 2024. Patients are 117% more likely to have an abnormal reading when transmitting a vital reading between 11 p.m. and 2:59 a.m., when access to care is most limited.

3. Delivering Rigor and Transparency
Cadence’s commitment to transparency is rooted in peer-reviewed studies and data-driven outcomes, helping to foster trust and accountability for both patients and providers over the long term. In the past year, our research has been peer-reviewed in esteemed academic journals such as the Journal of the American College of Cardiology and the Journal of Cardiac Failure. In October, the Peterson Health Technology Institute (PHTI) recognized Cadence’s Medication Management approach as a standout in digital hypertension care, validating our ability to achieve significant reductions in blood pressure compared to standard care. Wrapping up the year, in November, we presented groundbreaking new data on improved hypertension outcomes and total cost of care at the American Heart Association’s Sessions conference.
Some clinical highlights include:
- 1.5 million minutes spent interacting with patients in 2024, ensuring personalized, high-quality care.
- 230% increase in guideline-directed medical therapy for heart failure patients, adding an average of 5 years to their lives
- 107% increase in well-controlled blood pressure of patients on Cadence.
- 42% of Cadence’s diabetic patients achieved their blood glucose goal within 6 months of enrollment.
Perhaps most importantly, patients are feeling healthier and happier on Cadence. One recently shared with her Cadence Care Team: “It is amazing that being heart healthy is like a never ending loop of goodness – I improve my lifestyle, my heart gets healthier, my health improves and my quality of life improves. And the whole thing repeats itself. An amazing circle of goodness.”
4. Empowering Health Systems
At Cadence we believe that greater success can be achieved by working together with the country’s health systems. This year, we have grown our partnerships to 14 of the country’s leading health systems, allowing us to serve over 28,000 patients across 28 states.
The impact of Cadence’s solution extends beyond improved access to improved cost of care. For example, our analyses have shown that Cadence’s Remote Patient Care solution saves over $150 per hypertension patient per month and over $1,000 per congestive heart failure patient per month; a win-win for all parties.
Dr. Eve Cunningham, Chief of Virtual Care and Digital Health of Providence shared: "The Cadence program has helped us keep chronically ill patients safe at home to achieve the Quintuple Aim of healthcare improvement: Improve the patient experience by tailoring treatments and interventions to patient data; address population health through proactive, preventative care delivery; lower cost by connecting patients to the most appropriate care intervention and reduce avoidable hospitalizations; improve provider well-being by extending their reach via a virtual care team; and provide equitable care by reaching all patients, regardless of their ability to present in-person for treatment."
5. Easing Physician Burden
Integrated with all of the major electronic medical records, Cadence helps local providers extend their reach and provides real-time access to patient data so clinicians can easily identify candidates and place orders directly within their workflow. The Care Team is able to intervene when needed and address most patient issues without burdening physicians, so primary care physicians are able to practice at the top of their license and focus on their most acute cases.
Some highlights from the year include:
- 8.8 million vitals transmitted from home in 2024
- An average of 860,000 vitals per month
- Patient satisfaction of 4.9 out of 5
- Active patients are taking vitals 22 days each month, on average in 2024
In 2024, Cadence also expanded its services to include Chronic Care Management (CCM), equipping patients with a personalized care team to coordinate all of their health needs and support better health. From managing care plans to scheduling appointments and connecting patients with resources, Cadence’s Care Team has served as a trusted partner in navigating the healthcare system for its patients.
