Cadence Patient Stories: Cadence has helped me keep track of my health
In a new Patient Stories video released by Cadence, 73-year-old patient Jessie describes how Cadence's Remote Patient Monitoring (RPM) and virtual care program has helped her manage her chronic health condition and improve her life.
Jessie, who lives with her husband in Hickory, North Carolina, is battling congestive heart failure and was referred to Cadence by her primary care physician as part of Cadence's recently announced partnership with LifePoint Health.
In the video, Jessie details how Cadence has helped her to monitor her vital signs, manage her daily care, and ensure that she is staying on track on a daily basis. "I've had a wonderful experience with Cadence," Jessie said in the video. "They're all wonderful people. It's really nice to know there's somebody that's keeping track of you."
Cadence works with patients like Jessie who are dealing with four different types of chronic conditions: congestive heart failure, type-2 diabetes, hypertension or chronic obstructive pulmonary disease (COPD). Once a physician determines that a patient is a good fit for Cadence's RPM program, Cadence equips the patient with cellular-enabled devices and teaches him or her how to use the devices to monitor their blood pressure and weigh themselves daily from their own home.
"Cadence set my phone up for me," said Jessie. "They took my blood pressure and then showed me how it would go to my phone."
That data gets uploaded to Cadence's proprietary software platform and then reviewed by a team of Cadence clinicians, who work in partnership with the patient's physician to manage the patient's care. According to Jessie, Cadence has been simple and easy to use, even for those who may not be technologically savvy. "I would recommend Cadence to older people who may be leery of technology," she said.
At this point in her life, Jessie is eager to get healthy so she can get out on the road and enjoy time outdoors with her husband. "I want to be healthy for my family and for myself," she said. An avid motorcyclist, Jessie is looking forward to going on motorbike rides to Elvis Presley's mansion, Graceland, as well as to the California coast to see the Redwoods.
