Montage Health introduces new, innovative at-home patient monitoring service

The remote monitoring service advances quality and access of care for at-home patients with chronic conditions.

MONTEREY COUNTY, CA. (September 20, 2023) — Patients will soon benefit from improved quality and access of care with Montage Health Connected Care, an at-home patient monitoring service powered by Cadence that provides preventative care to patients with chronic conditions. The service will be provided at Montage Medical Group facilities for Medicare patients, lowering costs across the care continuum and helping providers deliver more efficient care.

Montage Health’s at-home patient monitoring program will initially support patients with hypertension, congestive heart failure, and type 2 diabetes, and will later expand to include other conditions.

“This program reduces emergency department trips because we are extending care into the patient’s home,” said Dr. Mark Carvalho, Chief Executive Officer, Montage Medical Group. “It is a win-win for patients and clinicians. Our patients will have access from home to an expanded care team. Our clinicians gain better visibility into disease progression and benefit from the support of a nurse- practitioner-led care team helping to keep patients safe.”

This high-level remote care is the next step in Montage Health’s commitment to exceptional value-based care that is personalized to meet each person’s individual goals and needs. The program combines dynamic remote monitoring technology with a nurse-practitioner-led clinical care team that collects, analyzes, and responds to a patient’s vitals in real time, supporting patients with medications, labs, and coaching to deliver exceptional care and optimal health. This is done through remote vitals-monitoring smart devices that patients use daily, paired with remote care phone conversations with a multidisciplinary healthcare team.

This type of care holds an impressive track record when it comes to improving health outcomes for patients. Patients with heart failure enrolled in Cadence are more likely to receive guideline-directed medical therapies, which have been shown to improve quality of life and reduce mortality[1]. Cadence has also supported health system partners to achieve significant increases in the percentage of patients who achieve blood pressure and glucose goals. In addition, patients enrolled in Cadence saw a 21% decrease in total cost of care [2].
